There are 5 steps to registering for a class:

         1. Complete the Registration form at the link below
         2. Click the Save the form and email it to Leigh at topnotchcanines.com
         3. Print the form and waiver
         4. Sign the Waiver (The waiver will be good for one year.) 
         5. Mail the Form, Waiver and Payment ($50 deposit) to
                       Debbie Strieter, 5201 W. Park View Lane Glendale, AZ 85310

 When signing up for a class, include as much information as possible concerning:
          what times and days you are available,
          the names of anyone you want to carpool or share a class with, and
          any information on any training your dog has already had. 
          for daytime requests - please include
                   the type of class you are interested in (private, semi-private, group),
         
It is not important to fill out the 1st, 2nd, and 3rd choice class boxes.  The information boxes that follow that will give you a place to provide me with input.  If your info doesn't fit please feel free to send a separate email to Leigh Fowler at Leigh at topnotchcanines.com  However, please do hit the submit button on the registration form so I can load your address etc into the database.
 
Due to an excessive number of last minute cancelations and those who simply do not show up for class,  we have adopted the following policy:
 
We require a $50 deposit with all class requests. The printed registration form and deposit must be postmarked within 72 hours of submitting your registration via email or your class request will be considered invalid.  If we are unable to fill your request for a class slot at a satisfactory date and time or you choose to be removed from the waitlist for any reason, your deposit will be returned. (Checks will be destroyed). 
 
Once you have been assigned to a class and confirmed via email or phone that the time and day is acceptable, you have 72 hours to postmark the final payment for class.  If payment is not recieved we reserve the right to offer your slot to another student. 
 
Deposits and final class fees are not refunded to students cancelling withing 1 calendar week of the start of class, unless we have someone on the waitlist ready to take your slot.
